What is On the Road to Living Well with Diabetes? |
On the Road to Living Well with Diabetes was developed in collaboration with University of Hawaii, University of New Mexico, Joslin Diabetes Center and USDA Cooperative State Research, Education and Extension Service. This six lesson series is taught by Washington State University Extension faculty and staff experienced in nutrition education.
- We offer the series of six - one and half hour classes in the community.
- The classes are targeted to people living with diabetes.
- The objective is to involve people in their own care and management of diabetes.
- Participants receive two tests related to diabetes management -
- A1c
- Blood Pressure
- Classes provide information about diabetes and the 5 tests to monitor diabetes care, and seek to empower individuals to become involved in their own care.
The program is intended to supplement instruction from qualified health care providers. Each class includes discussion, activities and problem solving segments. The program is available in English, Russian and Spanish.
How is the Program Delivered?
The series of lessons include:
- Getting on the Right Road reviews diabetes, and the five tests you need to know to take charge of the disease
- Take Charge of Your Diabetes focuses on using MyPyramid for healthy food choices and becoming more active as steps to take charge of diabetes. Participants are provided with skills to problem solve challenges and cope with lifestyle changes to manage diabetes
- Building a Helathy Meal Plan introduces the food guide pyramid for diabetes, label reading, the "Plate Method" for meal planning
- Building Heart Healthy Meals discusses the types of fat in relation to health, tips for reducing fat and the benefits of eating more fiber, vegetables and fruits
- Eating for Pleasure and Health provides basic strategies for eating healthy when away from home, and a discussion of the role of non nutritive sweeteners in foods.
All session incorporate physical activity. Paricipants receive a pedometer, stretch bands and exercise DVD.
A follow-up lesson at 3 months completes the series. At this time, participants receive the two tests, A1C and blood pressure again and complete surveys.
Funding
Funding is a cooperative venture between the federal, state, and local governments. Living Well with Diabetes is federally funded through the U.S. Department of Agriculture Cooperative State Research Education and Extension Service.
